When you submit the eligibility enquiry form on this website, we collect:

• First name
• Phone number
• Email address
• ZIP code (used to assess proximity to the trial location in Mantua, Ohio)
• Whether your horse is currently showing signs of an acute laminitis episode
• A brief description of observed symptoms (only if you choose to provide one)
• Your role (horse owner or referring veterinarian / farrier)
• Confirmation that you consent to be contacted

We do not collect payment information. We do not knowingly collect data from individuals under 18 years of age.

We collect and process your personal data on the basis of your explicit consent (Article 6(1)(a) and, where applicable, Article 9(2)(a) of the EU General Data Protection Regulation — GDPR), provided when you submit the form and tick the consent checkbox.

The purposes of processing are:

• To assess whether your horse may meet the preliminary eligibility criteria for the PTP-102 veterinary clinical trial
• To contact you to discuss the study and, if eligible, arrange a veterinary evaluation with Dr. Stachowski
• To comply with FDA Center for Veterinary Medicine (CVM) Investigational New Animal Drug (INAD) programme requirements

You may withdraw your consent at any time by emailing info@byrocktechnologies.com. Withdrawal will not affect the lawfulness of any processing carried out before the withdrawal.

We share your personal data only to the extent necessary for the clinical trial, with:

• Dr. Anthony Stachowski, DVM and his study team (Stachowski Alpacas, Mantua, Ohio) — to assess eligibility and arrange a veterinary evaluation
• The trial sponsor and study coordinators — as required for clinical trial administration and regulatory recordkeeping
• Regulatory authorities (including the FDA CVM) — as required by law for the conduct of an INAD study
• EmailJS — our email delivery service provider, used to transmit your form submission securely to our team

We do not sell, rent, or otherwise share your personal data with any third party for marketing purposes.

We retain your personal data for the duration of the trial enrollment period. If you do not proceed with study enrollment, your data will be deleted within 12 months of your initial enquiry.

Data relating to enrolled study participants may be retained for longer periods where required by applicable regulatory obligations (including FDA CVM recordkeeping requirements).
